CONSUMER-DIRECTED
CARE
Consumer-Directed Care is a philosophical orientation that emphasizes the ability of people with disabilities to
assess their own needs and make choices about what services would best meet their needs. It also reflects a view
that consumers can and should have options to choose the personnel who provide their care and to self manage over
the delivery and self-monitor the quality of such care. |

AGENCY-DIRECTED
CARE
Agency-directed care evolved principally out of the government funded programs that pay agencies to function as
the provider of record of caregiver services. In these arrangements, the agency recruit, a hire, trains, supervises
disciplines and terminates caregivers, and the agency sets the rates it charges patients and the wages it pays
its caregivers. The agency develops or participates in developing a patient's plan of care, and selects which caregiver
it will assign to render service to the patient. A patient's point of contact concerning the care is the agency. |

In the past, many families cared for their elderly loved ones at home, but as time goes on, it has become necessary
for family members to work outside of the home; it then becomes difficult to manage this on their own. Guardian
is a viable solution to that problem. Guardian offers in home care by providing basic companionship, conversation
and monitoring in one's own home.

• Prompting & Reminding of Medications
• Provide companionship and stimulation
• Meal planning, preparation, and clean up

non-emergency Dr's appointments.

a break. Our flexible schedule works with your needs. You pick the days, hours, or weeks that you need care. Caregivers
are available to assist you 24 hours a day, 7 days a week, including Holidays.
